What to bring to your appointment?

• ID: To confirm your identity, I'll need to see one of the following current (issued within the past 5 years) and valid government-issued photo IDs:

  • Florida driver's license or ID card

  • U.S. passport

  • Foreign passport stamped by U.S. Immigration and Naturalization Service

  • Driver's license or ID card from another U.S. state, territory, Canada, or Mexico

  • U.S. military ID

• Document: Bring the document you need notarized, completed and ready to sign.
